•  A completed application with a 5-year work history 
•  A valid state ID or Driver’s Licenses (if transporting clients)
•  Proof of Valid Car Insurance (if transporting clients)
•  Reliable Transportation 
•  Social Security Card
•  Current CNA certification/LPN license/RN license
•  CPR and First Aid Certification (no online certifications)
•  Negative TB skin test or chest x-ray
